Become a Patient
Establishing a relationship with a provider now is the best way to make sure they’ll not only be there for unexpected illnesses and injuries, but that they’ll be available to provide life-saving screenings and other services to help you maintain your health. When you elect to use Battenkill Valley Health Center as your primary care provider, notify your insurance company and call to make an appointment.
- Please call 802-375-6656 to reach a Patient Representative to set up an appointment with the provider of your choice for medical care and patient counseling at Battenkill Valley Health Center.

This includes your Authorization to Request Protected Health Information form which allows your medical records to be transferred to BVHC – this process can take at least 30 days.
- Plan to arrive to your first visit 15 minutes prior to your appointment and be sure to bring a photo ID and your insurance card.
- If you do not have insurance, fill out our Sliding Fee Program application that is included in the Patient Registration Packet. Our staff will work with you to understand all of your financial assistance options.
