The Steps to Establish Care at BVHC as a New Patient

Are you looking for a new doctor? Are you new to southern Vermont and need a doctor? BVHC can help! We are accepting new patients and welcome you to join our practice.

When it comes to choosing who cares for your health, you want someone who values and listens to you. At Battenkill Valley Health Center our primary care providers have one goal: to keep you and your family healthy.

Become a Dental and Medical Patient

If you wish to become a dental and medical patient at BVHC, simply fill out the New Med/Den Patient Registration Packet and call either office number to set up your first appointment. For your convenience, appointments can be made on the same day for both dental and medical if enough lead time is given. Call with any questions: dental (802) 430-7175; medical (802) 375-6566.

Become a Medical Patient


Establishing a relationship with a provider now is the best way to make sure they’ll not only be there for unexpected illnesses and injuries, but that they’ll be available to provide life-saving screenings and other services to help you maintain your health. When you elect to use Battenkill Valley Health Center as your primary care provider, notify your insurance company and call to make an appointment.

  • Please call 802-375-6656 to reach a Patient Representative to set up an appointment with the provider of your choice for medical care and patient counseling at Battenkill Valley Health Center.
  • Before your visit, please complete the New Patient Registration Packet. We ask that you return your completed packet prior to your first appointment. This includes your Authorization to Request Protected Health Information form which allows your medical records to be transferred to BVHC – this process can take at least 30 days.
  • Plan to arrive to your first visit 15 minutes prior to your appointment and be sure to bring a photo ID and your insurance card.
  • Fill out our Sliding Fee Program application that is included in the Patient Registration Packet. Our staff will work with you to understand all of your financial assistance options.

Become a Dental Patient

  • Please call 802-430-7175 to reach a Patient Representative to set up an appointment with Dr. Phillips and the dental team at Battenkill Valley Health Center.

It is Battenkill Valley Health Center’s mission to provide compassionate, responsive, comprehensive healthcare through a committed team of professionals ensuring affordable access to all. As such, BVHC participates with most commercial insurance plans, Vermont Medicaid, Medicare, and offers a sliding fee to uninsured and under-insured patients.
